Again, Dangerous Insurance

Every now and then our health insurance company decides that an in-network provider isn't, pays the out of network benefit, and we get a bill from said provider for the difference between what the insurance covers and what they charge. When you call them, of course, they're apologetic as can be, and fix the problem promptly. I am sure it's all an administrative hassle, but...after a couple of these incidents one does begin to wonder if there's someone at the insurer with his finger on the button that says "Not." If you catch them, they're gracious, but if you don't...oh well.
